Generic Semaglutide Is Cheap Now. Is It Quietly Costing You Muscle?
In short
yes, part of the weight that comes off on semaglutide is lean tissue, not only fat. Across trials it has ranged from roughly a quarter to two-fifths of the total. Protein and strength training are the two levers doctors point to, and your bathroom scale cannot tell you whether either one is working.

On 20 March 2026, Novo Nordisk's semaglutide patent expired in India. Within days, generic versions were arriving, and at least five Indian drugmakers undercut the original price by up to 80%. Monthly cost now runs from about ₹1,300 to ₹4,200, depending on brand, dose and format. For many families, "can we afford this?" suddenly has a different answer.

That makes a second question more important than the first: when the number on the scale drops, what exactly is leaving your body?

Cheap, but not casual

Before anything else, the rules did not get cheaper. On 24 March the Health Ministry warned that these drugs need a doctor's prescription and raised concern about easy availability through retail pharmacies, online platforms and wellness clinics. GLP-1 drugs are Schedule H medicines, and the Drug Controller General of India has inspected 49 businesses as part of a wider crackdown.

What the trials actually found about muscle

The best-known dataset is STEP 1: 1,961 adults on semaglutide 2.4 mg or placebo for 68 weeks, with average weight loss of about 15%. A smaller group of 140 people also had DXA body scans. In that group, total fat mass fell 19.3% and visceral fat fell 27.4%, which is the good news. Lean body mass also fell, by 9.7%.

Turn that into kilograms and it gets less comfortable. One review of the same data calculates that about 6.9 kg of the 15.3 kg lost was lean mass, close to 45%. Across trials, a Lancet Diabetes & Endocrinology commentary puts the lean share at 25% to 39% of the weight lost.

Three things stop this from being a scare story:

There is also a reassuring 12-month study called SEMALEAN. Lean mass dropped early (about 3 kg by month seven), then levelled off, and handgrip strength improved later in the year.

But here is the catch. In STEP 1, participants were counselled to reach 150 minutes of activity a week, such as walking. That was not enough to prevent the loss of lean mass. Which brings us to India.

Why this matters more here

There is a famous comparison in the medical literature, often called the Y-Y paradox: an Indian physician and a British one with the same BMI, and the Indian one carried more than twice the body fat percentage. Later DXA and impedance studies from India confirmed the pattern: high fat, low muscle.

An Indian consensus paper reports that sarcopenia (clinically low muscle mass or strength) affects between 14.2% and 39.2% of adults in different studies, and that "normal weight obesity", a normal BMI with too much fat, is common in South Asia and linked to higher sarcopenia risk.

Now add a policy angle. A public-health physician and a bioethicist, writing in STAT, argue that South Asians face greater cardiometabolic risk at lower BMI, and note that in already-lean people these drugs carry a muscle-loss risk that needs monitoring. Put those together and the concern is easy to see: someone starting at a lower weight, with less muscle to begin with, has a smaller cushion. (That last step is my reading of the evidence, not a trial result.)

Fix one: get honest about protein

The most detailed guidance so far is a 2025 joint advisory from four US bodies (ACLM, ASN, OMA and TOS). During major weight reduction, it says protein needs are based on 1.2 to 2.0 g per kg of adjusted body weight per day, or a simple absolute target of 80 to 120 g a day. It also admits the obvious problem: reduced appetite and taste changes make that hard to hit.

Now compare that with India's own numbers. ICMR-NIN sets the recommended intake at 0.83 g/kg/day, with average Indian intake estimated at 0.66 g/kg. Its 2024 guidelines also advise against protein powders for healthy people, note that intake above 1.6 g/kg does not add further muscle gain from resistance training, and warn about bone and kidney concerns with prolonged high-dose powders.

These do not contradict each other. The ICMR figure is for healthy people at a stable weight. The advisory is about people in active, medically driven weight loss. For a 70 kg adult, the first works out to about 58 g a day, the second to 80 to 120 g. Where you land is a call for your doctor or dietitian, especially if you have kidney disease.

That adds up to roughly 80 to 90 g. Two practical notes, based on common dietitian advice rather than a trial: if you can only manage a few bites, eat the protein on the plate first, and spread it across the day instead of trying to make one big meal do the work. If low appetite makes food targets unrealistic, a dietitian may still suggest a supplement. That is a decision to make with a professional, not off an influencer's discount code.

Fix two: protein without lifting is half a plan

This is the part people skip. The same advisory says plainly that more protein alone is likely inadequate to preserve muscle without structured resistance training. Walking is great for your heart and your step count (see how many steps you really need), but as STEP 1 showed, it did not stop lean mass from falling.

How strong is the evidence that strength training helps specifically on semaglutide? Honestly, it is still arriving. A trial called LEAN-PREP is testing exactly this in 232 adults on semaglutide or tirzepatide, comparing resistance exercise, extra protein, both, or neither over six months. The exercise arm is home-based: three sessions a week, building from one to three sets on major muscle groups. Results are not out yet. Until then, the case rests on evidence that tailored resistance training preserves lean mass during diet-driven and drug-driven weight loss.

A starter routine needs no gym: chair squats, incline push-ups against a wall or table, glute bridges and band rows, three times a week. Add a rep or a set as it gets easier. If you have joint problems, are older, or have other conditions, get a physiotherapist or qualified trainer to set it up.

The scale is a bad witness

Body weight is, in the words of one DXA study, a crude measure that does not separate fat, lean tissue and skeletal muscle. Two people can both lose 10 kg and end up in very different places.

If your doctor can arrange a DXA or body-composition scan at the start and again after some months, good. The same STAT essay points out that specialists and tests are not always available, so simple checks matter too: your waist measurement, how easily you stand from a low chair, how you feel climbing stairs, and grip strength if someone can test it. If those are getting worse while the scale improves, tell your doctor.

And when the injections stop?

In the STEP 1 extension, people who stopped semaglutide regained two-thirds of their lost weight within a year, though they stayed 5.6% below their starting weight overall. Most of the heart-and-metabolism improvements drifted back toward baseline.

The takeaway is not "never stop". It is that this is a long game, and the protein and strength habits you build now are the only part of the plan that does not depend on the injection.

Six questions to take to your doctor

  1. Am I actually a candidate at my BMI and waist size?
  2. Which brand and format are you prescribing, and is it approved in India?
  3. What daily protein target suits my weight, age and kidney health?
  4. Can we check body composition or grip strength now and again later?
  5. What do we do if nausea stops me from eating enough?
  6. What is the plan if or when we stop?

The bottom line

A cheaper injection changes the price of weight loss. It does not change the biology of it. The goal is not just a smaller number, it is less fat with your strength intact, and that takes a protein plan and a few weekly sessions of lifting something heavy.
